Medical Information & Forms

At
St. David's School, it is our desire to provide a safe and healthy environment
for learning. Your careful attention to the following forms helps us to meet this
goal effectively. All forms should be returned to the attention of the School Nurse
by
August 1 preceeding the start of the school year.

Who? Every student, every year. Please include
all current medications, even those not taken during the school day.
Be sure to check your preference regarding over the counter medications provided
in the school infirmary.

Who? All new students (including K). All returning 5th
and 9th graders All students who wish to participate in athletics. Athletic physicals
expire 12 months after the last physical exam. Athletes with expired physicals will
not be permitted to try out, practice, or play.

Who? All students who will require medication during the
school day. This includes medications for asthma, diabetes, or other chronic conditions.
If the medication is a prescription, a doctor's signature is required. These medications
must be brought to the school nurse on or before the first day of school, unless
the student has been approved by parent and physician to self medicate (as in the
use of an asthmatic inhaler). The form is required whether the medication is self
administered or given by the nurse.
Please note that the MEDICATION RELEASE FORM is also required in the event that
your student develops the need for medication at school for a temporary situation.
Please be aware that NO MEDICATION will be dispensed to your child without your
written authorization. If prescription medication, your doctor's signature will
also be required. These forms are available in the infirmary at all times, or your
doctor can use one from his/her office at the time that you receive the prescription.
